Looking to outsource a web form that will post data to MySQL, forward the same data via email to an internal contact as well as autoreply to the enduser with one or two file attachments.\r\n\r\nSpecs:\r\n\r\nLanguages:\r\nFLASH / PHP or HTML / CGI\r\n\r\nDatabase\r\nMySQL\r\n\r\nFiles to be emailed\r\nZIP\r\n\r\nDetailed Specs:\r\n\r\nA form, listing pertinant contact info (ie: name, title, company, contact phone, contact email) as well as the option to select (via radio button) A, B (send me more information about A. Coffee, B. Beer) or both. Once submitted, data will post to a MySQL database as well as forward an email to marketing. Furthermore, an autoreply will be generated sending an email to the registrant/end user with file attachments with information pertaining to A, B, or both.\r\n\r\nI have attached a sample image of the form for your review.\r\n\r\nI look forward to a prompt reply.
